St. Vincent’s endeavors to provide excellent clinical and compassionate care to all we serve. If, however, we have not met your expectations and you have a concern about our quality of care or patient safety, please let us know.

Teamwork and communication
Our staff places considerable emphasis on improving teamwork and communication between each other and all of our patients and family members. Every weekday morning, members of the management team “huddle” in the library to discuss safety concerns and issues that have occurred during the past 24 hours. Additionally, unit-specific huddles are convened throughout the week to address safety and patient concerns on a particular floor or in a specific area of the hospital.
Ensuring best possible outcomes
In addition to our own internal efforts to improve quality and safety, the Centers for Medicaid and Medicare Services and The Joint Commission request that hospitals collect and provide information on how patients with certain medical and surgical diagnoses are treated. This data is publicly reported. The State of Connecticut was the first to achieve 100% participation by its hospitals nationally in this voluntary program, with St. Vincent's being one of the first participating Connecticut hospitals to participate.

Please note that due to the complexity of the data collection requirements, the information reported by the Centers for Medicare and Medicaid Services (CMS), and The Joint Commission (TJC), reflects a hospital's performance in these areas seen six months to one year ago.
A High Reliability Organization
St. Vincent's is proud to be the innovator in the state of Connecticut and the first Connecticut hospital to become a High Reliability Organization, placing patient safety above all else. The Medical Center continues to partner with Healthcare Performance Improvement (HPI) in its effort to create error prevention tools and programs and to achieve a standard of zero preventable errors.
With the help of HPI, St. Vincent's has learned about high reliability from organizations outside health care that have made their business safe. HPI performed benchmarking at St. Vincent's to determine targeted areas in need of the most improvement, and is also quantified the 'failure modes" behind each error, including lapses in critical thinking, compliance or lack of competency.
